摘要
加速康复外科(enhanced recovery after surgery,ERAS)是指采用有循证医学证据的围术期处理的一系列优化措施,旨在减少或降低手术患者心理和生理的创伤和应激反应,促进患者术后快速康复。ERAS在诸多外科领域已取得可观的效果,具体措施包括术前6小时禁食,选择微创方法,科学的液体管理,避免使用或尽早拔出各种引流管和插管,尽早下床活动及术后当天尽快恢复正常饮食。ERAS可缩短患者住院天数,降低并发症发生率和再入院率,减少住院费用。尽管目前ERAS的术后评价主要来自结直肠手术,但其在泌尿外科也有较广阔的应用前景。由于ERAS可改善临床结局,节约医疗资源,使其成为今后外科发展的重要方向。
Enhanced recovery after surgery(ERAS) is a series of optimized measures for perioperative management with evidence-based medical evidence that aims to reduce the psychological and physiological traumatic stress reactions in the surgical patients, so as to promote rapid postoperative rehabilitation. ERAS has achieved improved outcomes in many surgical procedures. Specific elements include preoperative fasting 6 hours before surgery, minimally invasive approaches, management of fluids, avoiding the use of drains and tubes or removing them as soon as possible, early mobilization, and food intake at the day after surgery. ERAS protocols have resulted in a shorter length of hospital stay, lower complication rates, readmission rates and hospital costs. The initiation of ERAS is mainly associated with colorectal surgery but has a great prospect in the department of urology. Implementation of ERAS results in major improvements in clinical outcomes and reducing costs, making ERAS an important example of value-based care applied to surgery.
