摘要
目的探讨从医院到社区无缝隙护理管理模式在慢性心力衰竭患者中的应用,给临床护理提供理论依据。方法选取2015年5月—2016年5月在医院接受治疗的80例慢性心力衰竭患者作为研究对象,根据给予的护理方法进行分组,对照组的40例患者给予常规护理,观察组的40例患者按照从医院到社区无缝隙护理管理模式进行护理,比较两组患者护理效果。结果观察组的社会能力、躯体功能、生活质量和情绪功能的评分均明显高于对照组;观察组患者的SAS评分和SDS评分均明显低于对照组,治疗依从性评分高于对照组,;观察组患者的各项心功能指标均明显优于对照组(P<0.05),6 min步行试验结果高于对照组(P<0.05)。结论在慢性心力衰竭中应用医院到社区无缝隙护理管理模式,能明显改善患者的心功能,提高治疗依从性,提高生活治疗,值得临床推广应用。
Objective To study the application of seamless nursing management model from hospital to community for pa- tients with chronic heart failure and provide theoretical basis for clinical nursing. Methods 80 cases of patients with chronic heart failure admitted and treated in the hospital from May 2015 to May 2016 were selected as the research objects and di- vided into two groups with 40 cases in each, the control group adopted the routine nursing, while the observation group adopted the seamless nursing management model from hospital to community for nursing, and the nursing effect of the two groups was compared. Results The social ability, body function, quality of life and emotional function scores in the observa- tion group were obviously higher than those in the control group, and the SAS score and SDS score in the observation group were obviously lower than those in the control group, and the treatment compliance score was higher than that in the control group, and various heart function indexes in the observation group were obviously better than those in the control group（P〈 0.05）, and the 6-minute walk test result was higher than that in the control group （P〈0.05）. Conclusion The application of seamless nursing management model from hospital to community for patients with chronic heart failure can obviously im- prove the heart function of patients, improve the treatment compliance and improve the life treatment, which is worth clini- cal promotion and application.
