摘要
目的探讨多层螺旋CT(MSCT)在进展期胃癌T分期中的应用价值。方法经术后病理结果证实为进展期胃癌的126例患者作为研究对象,所有患者均于术前1周内做MSCT检查,结果由两位有经验的放射科医生确定,以常规病理结果为标准,将所得MSCT检查结果与术后病理常规结果进行对比分析。结果共有104例进展期胃癌患者经MSCT正确诊断,符合率为82.54%。病理分期中的T1期、T2期、T3期在MSCT中均有诊断过低或者过高情况发生,但均有较好的符合度,其中T1期符合度为63.64%、T2期符合度为86.49%、T3期符合度为82.26%。在T4期诊断中,出现2例过低诊断为T3,其符合度为87.50%。经Kappa一致性检验,发现MSCT在不同分期的进展期胃癌中均与病理结果具有良好的符合度。结论 MSCT对进展期胃癌的检出率高,可作为进展期胃癌术前分期的首选检查。
Objective To investigate the value of muhislice CT on T stage in advanced gastric carcinoma. Methods A total of 126 patients with advanced gastric carcinoma confirmed by postoperative pathology were selected. The muhislice CT examination was taken at one week before surgery, and the examination results were given by two experienced radiologists. The examination results of multislice CT and conventional pathology resuhs which as gold standard were compared. Results A total of 104 patients diagnosed by muhisliee CT were correct, the coincidence rate was 82.54%. The coincidence rate of multislice CT to diagnosing T1, T2, T3 were 63.64%, 86.49%, 82.26% respectively. In diagnosis of T4 stage, two cases were mistake diagnosed T3, which the coincidence rate was 87.50%. Kappa consistency test showed that muhislice CT had good coincidence rate in different stages of advanced gastric carcinoma with the pathology results. Conclusion Multislice CT had high detection rate in advanced gastric carcinoma, which can be as the first choice for preoperative stage of ad- vanced gastric carcinoma.
