孕产妇红细胞各项参数、锌原卟啉、血清铁的检测分析 被引量:5

摘要 目的:了解妊娠期间红细胞各项参数、锌原卟啉、血清铁在不同孕期的变化规律,为孕期贫血的诊断提供依据。方法:采用全血细胞分析仪、国产ZPP-3800型血液锌原卟啉测定仪、全自动生化分析仪测定孕妇红细胞各参数、锌原卟啉及血清铁水平。结果:随孕周的增加,红细胞、血红蛋白、红细胞比积、平均红细胞体积、平均红细胞血红蛋白含量下降(P<0.01),红细胞分布宽度、锌原卟啉增高(P<0.01)。平均红细胞体积、平均红细胞血红蛋白含量以产后组为最低(P<0.01),红细胞分布宽度以产后组为最大(P<0.05)。孕晚期血清铁明显低于孕早期和孕中期(P<0.01)。结论:孕晚期血清铁下降明显,红细胞有向小细胞低色素发展的趋势,因此,在怀孕的中、晚期,应及时补充铁剂,防止缺铁性贫血的发生。 Objective: To understand the change regularities of red blood cell parameters, zinc protoporphyrin and serum iron during different periods of pregnancy, and provide a basis for diagnosis of anemia during pregnancy. Methods: Hematology analyzer, domestic ZPP - 3800 blood zinc protoporphyrin hematofluorometer and fully automatic biochemical analyzer were used to detect the levels of red blood cell parameters, zinc protoporphyrin and serum iron. Results: With increase of gestational week, red blood cell count, hemoglobin level, hematoerit, mean corpuscular volume and mean corpuscular hemoglobin content decreased (P 〈 0. 01 ), red blood cell distribution width and zinc protoporphyrin level increased (P 〈 0. 01 ) . Mean corpuscular volume and mean corpuscular hemoglobin content in postpartum group were the lowest ( P 〈 0. 01 ), while red blood cell distribution width in postpartum was the highest (P 〈 0. 05) . Serum iron level during the third trimester of pregnancy was statistically significantly lower than those during the first and the third trimesters of pregnancy (P 〈 0. 01 ) . Conclusion : Serum iron level during the third trimester of pregnancy decreases significantly, there is a trend of turning to smaller and low hemoglobin in red blood cells ; thus, timely iron supplement is necessary to prevent the occurrence of iron - deficiency anemia.
