摘要
目的 探讨手术治疗Hawkins Ⅲ、Ⅳ型距骨颈骨折的临床疗效。方法 手术治疗27例Hawkins Ⅲ、Ⅳ型距骨颈骨折患者。结果 患者均获随访，时间9～24个月。解剖复位9例，接近解剖复位15例，位置较差3例。出现距骨缺血性坏死5例。无踝关节皮肤坏死和骨折不愈合。采用Hawkins疗效标准评价：优8例，良8例，可6例，差5例。结论 Hawkins Ⅲ型和Ⅳ型距骨颈骨折只要掌握手术时机，正确选择手术人路和内固定置人物，能有效降低距骨缺血坏死的发生率，减少术后并发症，最大限度保留踝关节功能。
Objective To explore the surgical treatment of Hawkins Ⅲ and Ⅳ type clinical curative effect of talus neck fracture. Methods 27 cases Hawkins Ⅲ and Ⅳ type talus neck fractures were treated with operation. Results All patients were followed up for 9 ～24 months. Anatomical reduction was attained in 9 cases, close to anatomical reduction in 15 cases, poor reduction in 3 cases, ischemic necrosis of talus was recorded in 5 cases, no ankle skin necrosis was found and all fractures got healing. According to Hawkins evaluation criteria, 8 cases were excellent, 8 good, 6 fair, and 5 poor. Conclusions Hawkins Ⅲ and Ⅳ type talus neck fractures as long as master the operation time, correct selection of surgical approach and internal fixation implant can effectively reduce the incidence of ische-mic necrosis of talus and the postoperative complications, maximize the ankle joint function.
