脂肪干细胞辅助的自体脂肪移植隆乳术临床疗效分析 被引量:15

摘要 目的:评价脂肪干细胞(ADSCs)辅助的自体脂肪移植隆乳术的临床效果。方法:选取2008年11月—2009年12月于韩国首尔TIARA整形外科医院接受ADSCs辅助的自体脂肪移植隆乳术的患者100例,对其临床效果进行研究和分析。所有患者均自下腹或大腿部抽取脂肪组织,从其中100 mL脂肪组织中提取ADSCs约5 mL,与填充用脂肪组织均匀混合后注射于患者乳房内,每侧乳房注射约200mL脂肪。术后测量患者直立位乳头根部至前胸壁平面的垂直距离即为乳房的隆起值。测量术后2周、1个月、3个月和6个月患者乳房的隆起值,与术前隆起值之差即为隆起增加值,并以2周时的隆起值(或隆起增加值)作为基准,评价乳房隆起的保持效果。结果:100例患者乳腺整形手术均顺利完成,未出现严重的并发症。绝大多数患者对隆乳效果表示满意,尤其对于自然的胸部曲线与触感非常满意。术后2周测量100例患者隆起的总值是1 435 mm,平均隆起14.35 mm;术后1个月保持率为79.08%,术后3个月保持率为66.75%,术后6个月保持率为70.57%。术后1、3和6个月的隆起值与术后2周相比,差异均有统计学意义(P<0.001);术后1个月隆起值与术后3、6个月比较,差异有统计学意义(P<0.05);而术后3个月和6个月之间差异无统计学意义(P>0.05)。不同年龄组的患者术后隆乳效果除术后2周有差异外(P<0.05),其他时间段的差异无统计学意义(P>0.05)。结论:ADSCs辅助的自体脂肪移植隆乳术临床效果确切可靠。术后3个月乳房内保存的移植脂肪基本成功存活,之后乳房的隆起效果不再发生变化。不同年龄组的患者均可以通过ADSCs辅助的自体脂肪移植隆乳术获得满意的临床效果。 Objective- To evaluate the clinical outcomes of adipose-derived stem cells assist ed fat transplantation. Methods: One hundred patients was enrolled in this research who underwent ADSCs assisted fat transplantation for breast augmentation from November 2008 to December 2009. Adipose tissue was suctioned subcutaneously from lower abdomen or thighs of the patients, and 5 mL of ADSCs was abstracted from 100 mL of liposuction aspirates. ADSCs was mixed uni- formly with graft material and injected to patients' breast, each one for about 200 mL. The vertical distance was measured between root of nipple and sternal coronal plane before and after surgery, taking the increasing amount to measure augmentation of breast. It was measured 2 weeks, 1 month, 3 months and 6 months after surgery, and retention percentage was calculated using 2 weeks' measurement as reference value. Results: Each surgery was completed successfully without severe complication, and almost all the patients were satisfied with the soft and natural-ap- pearing augmentation. Average increase of vertical distance between root of nipple and sternal coro- nal plane was 14.35 mm, and the retention percentage of 1, 3 and 6 months after surgery was 79.08%, 66.75% and 70.57%, respectively. Differences between 2 weeks and the other three groups, between 1 month and 3 months, 6 months showed significance (P〈0.05). And there was no significant difference between 3 months and 6 months after surgery. Difference among differ- ent age groups shows no significance except 2 weeks after surgery. Conclusion: ADSCs assisted fat transplantation for breast augmentation is safe and effective, especially excellent in the natural ap- pearing. Fat tissues retained in breast for3 months after surgery would survive and maintain the augmen- tation of breast for long time. Patients of all age groups may s hare the sa me benefit of this technology.
