摘要
目的:综合评价糖尿病视网膜中央动脉(CRA)和眼动脉(OA)血流动力学改变对糖尿病性视网膜病变(DR)的诊断意义。方法:计算机网上检索中国期刊全文数据库1994~2006年来有关CRA和OA彩色超声多普勒血流动力学测定的DR病例-对照研究文献33篇对CRA和OA血流参数进行同质性检验和合并效应量的检验。结果:合并分析后有统计学意义的同质性研究结果为糖尿病患者的OA平均血流速度比正常人低0.26~0.31标准差;DR出现前,OA收缩期峰值血流速度比正常人低0.24~0.3标准差,OA舒张末期血流速度比正常人低0.31~0.37标准差;单纯型DR的OA舒张末期血流速度比正常人低0.94~1.01标准差,也比DR出现前糖尿病患者低0.56~0.67标准差;增殖型DR的CRA平均血流速度比正常人低2.98~3.21标准差,也比单纯型DR者低0.49~2.43标准差。结论:糖尿病患者表现早期OA的血流速度降低,DR增殖期CRA的平均血流速度降低。血流动力学改变早于DR肉眼可见的视网膜形态学改变。
AIM: To estimate diagnostic effects of hemodynamic changes of ocular artery (OA) and central retinal artery (CRA) in diabetic retinopathy (DR) in China.
METHODS: Hemodynamic parameters from 33 research aP ticles from 1994 to 2006 were cited from National Knowledge Infrastructure (China) searching and analyzed with metaanalysis in homogeneity test and combined test.
RESULTS: After the tests, homogeneous research results of the parameters included the decrease of mean velocity (0.260.31 Standard Deviation, SD) of OA in diabetes mellitus, peak systolic velocity (02403 SD) and end diastolic velocity (0.310.37 SD) of OA in diabetes without DR, end diastolic velocity (0.941.01 SD) of OA in background DR, and mean velocity (2.983.21 SD) of CRA in proliferative DR compared with the normal. Meanwhile, the end diastolic velocity of OA in background DR was also lower (0.560.67 SD) than that in diabetes without DR. The mean velocity of CRA in proliferative DR was lower (0.492.43 SD) than that in background DR.
CONCLUSION: In early stage of diabetic or DR, blood flow of OA was decreased without regarding to the gender, age, or course of diabetics, and the mean velocity of CRA in proliferative DR was decreased. The hemodynamic changes can be detected before retinal pathological changes of DR.
