摘要
目的观察奥曲肽在治疗肝硬化门脉高压症过程中对心率的影响。方法对301例住院的肝硬化门脉高压症合并食管胃底静脉曲张破裂出血患者进行观察,分成奥曲肽组和对照组。对两组患者在治疗前、治疗后24,72 h,1周和停药后72 h内的血压和心率,通过心电监护仪进行监测。结果两组患者病情的严重程度差异无显著性,治疗前后两组之间的血压及治疗前、治疗后24 h时两组之间的心率均无差异,而治疗后72 h及1周时奥曲肽组的心率均明显低于对照组,停药48 h后心率恢复到用药前水平。结论奥曲肽可明显降低肝硬化门脉高压症患者的心率,心率降低可能是本品降低门脉压力的机制之一,但奥曲肽降低心率的机制尚未明确,有待于进一步探讨。
OBJECTIVE To observe the influence of octreotide on the heart rate of cirrhosis patients with portal hypertension. METHODS 301 esophaged-gastric varices bleeding with portal hypertension were divided into octreotide group (185 cases) and control group (116 cases). The blood pressure and heart rate were observed by ECG monitor at five steps (before treatment, 24 h, 72 h, 1 week after treatment and 72 h after stopping administration). RESULTS The situation was similar in both octreotide group and control group. Before and after treatment the blood pressure were similar in both groups. The heart rate was similar in both groups before treatment and 24 h after treatment. However, 72 h and 1 w after treatment the heart rate in octreotide group were significantly lower than that of control group. CONCLUSION The heart rate of patients with cirrhosis portal hypertension may be significantly reduced by octreotide. The reduced heart rate was probably one of the mechanisms to decrease the portal pressure via octreotide. The reduce mechanism of heart rate caused by octreotide was unclear and should be further studied.
