摘要
目的无创正压通气(NIPPV)治疗老年慢性阻塞性肺疾病(COPD)并II型呼吸衰竭的临床价值。方法住院老年COPD并呼吸衰竭62例,其中对照组30例,用常规治疗。治疗组32例,除常规治疗外,用口鼻面罩进行NIPPV。结果NIPPV治疗1d后pH、PaO2上升及PaCO2下降(P<0.05);对照组治疗3d后pH、PaO2值上升及PaCO2下降(P<0.01)。治疗组呼吸衰竭纠正时间及住院时间缩短且呼吸衰竭缓解率高(P<0.05)。结论早期合理的应用NIPPV可以纠正呼吸肌疲劳,提前改善症状及血气分析结果,是治疗老年COPD并呼吸衰竭的重要手段。
[Objective] To evaluate the efficacy of noninvasive positive pressure mechanical ventilation（NIPPV） on chronic obstructive pulmonary disease（COPD） elderly patients with type Ⅱ respiratory failure. [Methods] 62 elderly cases were involved randomly in the study. 32 cases in the treatment group were administered NIPPV support via facial mask using Esprit machine and 30 cases were given to treat with the routine therapy as the control group. All patients were treated with antibiotics and other traditional therapy. [Results] One day after using NIPPV for the treatment group, PH and PaO2 values were markedly increased with PaCO2 reduced （P 〈0.05）; the patients in the control group had gained similar results for nearly three days. For the treatment group, there was a shorter time and higher curative rate of changing respiratory failure with a significant reduction in in-hospital day （P 〈0.05）. [Conclusion] An earlier use of NIPPV can greatly improve anoxia and decrease hypercapnia, which might play an important role in the treatment of COPD patients with type Ⅱ respiratory failure.
