摘要
目的探讨急性心肌梗死(Acute Myocardial Infarction,AMI)患者碎裂QRS波(fQRS波)、QRS-T夹角及QT间期离散度与Gensini积分的相关性。方法选取2020年12月至2021年12月于我院行冠脉造影检查确诊AMI的109例患者为AMI组,并根据Gensini积分分为低度、中度及高度狭窄亚组,同期选择我院住院的非AMI患者60例作为对照组,两组心电图进行对比分析。根据是否存在fQRS波将AMI患者分为两组,比较其QRS-T夹角值和QT间期离散度。应用Pearson模型分析心电图指标与Gensini积分的相关性。结果急性心肌梗死组的fQRS发生率、QRS-T夹角值及QT间期离散度均大于对照组(P>0.05);冠脉病变程度与fQRS波发生率、QRS-T夹角值、QT间期离散度有统计学意义(P<0.05);fQRS组的QRS-T夹角值及QT间期离散度均大于无fQRS波组(P<0.05);fQRS波、QRS-T夹角、QTd均与Gensini积分呈正相关(P<0.05)。结论Gensini积分与心电图碎裂QRS波、QRS-T夹角及QT间期离散度存在相关性。
Objective To investigate the correlation between fragmented QRS wave,QRS-T angle,QT interval dispersion and Gensini score in patients with acute myocardial infarction(AMI).Methods All 109 patients with AMI diagnosed by coronary angiography in our hospital from December 2020 to December 2021 were selected as the AMI group,and were divided into low,moderate and high stenosis subgroups according to Gensini score.All 60 patients without AMI hospitalized in our hospital at the same time were selected as the control group.The ECG of the two groups were compared and analyzed.AMI patients were divided into two groups according to whether there were fragmented QRS waves,and their QRS-T angle values and QT interval dispersion were compared.Pearson model was used to analyze the correlation between ECG indexes and Gensini score.Results The incidence of fQRS,QRS-T angle and QT interval dispersion in AMI group were higher than those in control group(P>0.05).The degree of coronary artery disease was significantly related to the incidence of fQRS wave,QRS-T angle and QT interval dispersion(P<0.05).The QRS-T angle and QT interval dispersion in the fQRS group were higher than those in the non fQRS group(P<0.05).The fQRS wave,QRS-T angle and QTd were positively correlated with Gensini score(P<0.05).Conclusions Gensini score is correlated with fQRS wave,QRS-T angle and QT interval dispersion.
