摘要
目的测定类风湿关节炎(RA)患者血清中MMP-3水平,并分析其与RA患者病情活动性及骨破坏的关系。方法选择就诊我院的RA病人80例,其中进一步分为RA活动组和RA非活动组,有骨破坏组和非骨破坏组,采用ELISA法检测80例初诊的RA患者和20例健康体检者(健康对照组)血清中MMP-3水平。分析MMP-3水平在不同组别的表达及其与RA患者临床、炎性指标及关节放射学评分、关节功能评分的相关性。结果 1RA活动组与RA非活动组及健康对照组比较,差异均有统计学意义(P<0.05),RA非活动组血清中MMP-3水平高于健康对照组,差异有统计学意义(P<0.05);2RA患者骨破坏组(指放射学检查正常)与RA患者非骨破坏组比较,差异有统计学意义(P<0.05);RA患者非骨破坏组与健康对照组比较,差异有统计学意义(P<0.05);3RA患者血清中MMP-3水平与受累关节数目、DAS28、血沉、C-反应蛋白、RF、关节放射学评估分数、关节功能评分均呈正相关关系(r分别为0.295,0.489,0.599,0.750,0.401,0.512,0.405,P<0.05)。结论 RA患者血清MMP-3水平明显升高,可作为RA的活动性指标及骨关节侵蚀的预测指标。
Objective To test the serum level of MMP-3 in patients with rheumatoid arthritis（ RA） and analyze its relationship with disease activity and bone erosions. Methods The 80 RA patient who were initially diagnosed and treated in hospital affiliated to weifang medical university were identified. Patients of RA group were further divided into active RA group and non-active RA group,bone erosion group and without bone erosion group. Then,Detected the serum level of MMP-3 in this 80 RA patients（ RA） and 20 healthy physical examination（ healthy control group） by ELISA method. Analysis of MMP-3 levels in different groups of expression and RA patients clinical radiology,inflammatory indices and joint score,joint function score. Results 1There was a significant difference between active RA group and nonactive RA group and healthy control group,the differences were statistically significant（ P〈0. 05）,there was difference between non-active RA group and healthy control group（ P〈0. 05）; 2There was a significant difference between bone erosion group（ radiology is normal） and without bone erosions group（ P〈0. 05）,there was difference between without bone erosions group and healthy control group（ P〈0. 05）; 3the levels of serum MMP-3 in RA patients were positive related with disease activity index,including DAS28,ESR,CRP,RF,radiological assessment scores and Joint function score（ r = 0. 295,r = 0. 489,r = 0. 599,r = 0. 75,r = 0. 401,r = 0. 512,r = 0. 405,P〈0. 05）. Conclusion RA patients significantly increased serum levels of MMP-3,can be used as the activity of the RA index and bone erosion prediction index.
