食管鳞癌患者血清15种非必需微量元素定量分析 被引量:7

Quantitative analysis of 15 kinds of non-essential trace elements in the serum of patients with esophageal squamous cell carcinoma

摘要 目的研究食管鳞癌患者血清中多种非必需微量元素的变化情况。方法采用频数匹配(匹配因素为年龄和性别)的病例对照研究,采集2009-03-31-2010-03-31河南省安阳市肿瘤医院收治的食管鳞癌患者及安阳农村地区的健康对照的血清样本,应用电感耦合等离子体质谱仪定量检测其血清中15种非必需微量元素浓度,采用非条件Logistic回归估计各元素的不同水平对食管癌患病风险的影响,采用Spearman相关分析研究元素间相关性。结果共纳入100例患者和100名对照。非条件Logistic回归分析结果显示,以中浓度组为参照组,未调整其余因素(性别、年龄、吸烟史、饮酒史和食管癌家族史)时,血清铋(Bi)、汞(Hg)、铊(Tl)和铀(U)的高浓度组,以及硼(B)、钛(Ti)、锗(Ge)、铷(Rb)、铯(Cs)和钍(Th)的低浓度组,其食管癌患病风险升高,P<0.05;血清Bi低浓度组食管癌患病风险降低,P<0.05。而锂(Li)和饰(Ce)低浓度和高浓度组的食管鳞癌患病风险均升高,P<0.05。调整上述因素后,血清Bi、Hg、Tl和U高浓度组,以及B、Ti、Ge、Rb、Cs和Th低浓度组,其食管癌患病风险升高,P<0.05;血清Bi低浓度组,以及La高浓度组,其食管癌患病风险降低,P<0.05。Li和Ce低浓度和高浓度组的食管鳞癌患病风险均升高,P<0.05。相比于对照组,病例组元素间相关关系较对照组有所减弱,且部分元素间的相关性与对照组相比发生了反转。结论人体内多种血清非必需微量元素含量的变化与食管鳞癌的发生和发展密切相关。食管鳞癌患者血清中部分有害元素越高,有抗癌活性的有益元素越低,则食管鳞癌患病风险越高。 Hospital;Centre of Medical and Health Analysis,Peking University; OBJECTIVE To study the change of multiple non-essential trace elements in the serum of patients with esophageal squamous cell carcinoma(ESCC).METHODS A frequency-matched case-control study was performed.We collected serum samples of patients with ESCC from Anyang Cancer Hospital during 2009-03-31 and 2010-03-31,and of controls from Anyang rural area.The inductively coupled plasma mass spectrometry was used to quantitatively detect 15non-essential trace elements in their serum.Unconditional logistic regression model was applied to estimate the effect of different element levels on the risk of esophageal cancer,and spearman correlation analysis was used to calculate the correlation among the elements.RESULTS A total of 100 ESCC patients and 100 healthy controls were enrolled.The result by unconditional logistic regression indicated that when unadjusted for age,sex,alcohol drinking,cigarette smoking and family history of ESCC,groups with high levels of serum Bi,Hg,Tl and U,and low levels of B,Ti,Ge,Rb,Cs and Th had increased risk of esophageal cancer(P〈0.05)compared with the middle level group.Individuals with low levels of Bi had decreased risk of esophageal cancer(P〈0.05).The esophageal cancer risk was significantly increased for groups with both low and high levels of Li and Ce(P〈0.05).When adjusted for above factors,groups with high levels of serum Bi,Hg,Tl and U,and low levels of B,Ti,Ge,Rb,Cs and Th had increased risk of esophageal cancer(P〈0.05).Individuals with low levels of Bi and high levels of La had decreased risk of esophageal cancer(P〈0.05).The esophageal cancer risk was significantly increased for groups with both low and high levels of Li and Ce(P〈0.05).The correlation among elements of the case group was weakened,and the correlation of some elements reversed compared to the control group.CONCLUSIONS Multiple non-essential trace elements in the serum are closely related to the occurrence and dev
