小剂量地塞米松对创伤性脑损伤合并海水淹溺大鼠脑水肿的影响

Effects of low-dose dexamethasone on cerebral edema in rats with traumatic brain injury complicated by seawater drowning

摘要 目的观察地塞米松(DX)对创伤性脑损伤(TBI)合并海水淹溺(SWD)大鼠脑水肿的干预效果。方法按照随机数字表法将24只SD大鼠分为假手术组、TBI+SWD组、DX治疗组,每组8只。采用TBI+SWD复合模型,DX治疗组大鼠腹腔注射1 ml的DX(1 mg/kg)。对各组大鼠进行改良神经功能缺损评分(mNSS)和脑组织含水量检测,HE染色观察大鼠海马组织病理改变,免疫组化染色观察大鼠海马组织水通道蛋白4(AQP4)的表达情况。结果与假手术组相比,TBI+SWD组、DX治疗组大鼠的mNSS评分及脑组织含水量明显升高,差异有统计学意义(P<0.05);与TBI+SWD组相比,伤后24 h DX治疗组大鼠的mNSS评分明显下降,差异有统计学意义(P<0.05);与TBI+SWD组相比,DX治疗组大鼠伤后各时间点的脑组织含水量差异无统计学意义(P>0.05)。DX治疗组大鼠较TBI+SWD组伤后各时间点神经元细胞核固缩、细胞肿胀、组织基质疏松均有改善。TBI+SWD组与DX治疗组大鼠伤后各时间点均可见海马组织AQP4阳性表达。结论低剂量DX可以改善TBI+SWD大鼠的脑水肿反应,但脑保护作用可能有限。 Objective To observe the effects of dexamethasone(DX)on cerebral edema in rats with traumatic brain injury(TBI)complicated by seawater drowning(SWD).Methods SD rats were randomly divided into sham group,TBI+SWD group,and DX treatment group,according to the random number table method..TBI+SWD composite model was used.The rats in the DX treatment group were intraperitoneally injected with 1 mL DX(1 mg/kg).The modified neurological severity score(mNSS)and brain water content were detected.The pathological changes of hippocampus were observed by HE staining,and the expression of aquaporin 4(AQP4)in hippocampus was observed by immunohistochemical staining.Results Compared with the sham group,the mNSS scores and brain water content of the TBI+SWD group and the DX treatment group were significantly increased(P<0.05).Compared with the TBI+SWD group,the mNSS scores of the DX treatment group decreased significantly at 24 h after injury(P<0.05).Compared with the TBI+SWD group,there was no significant difference in brain water content at each time point after injury in the DX treatment group(P>0.05).Compared with the TBI+SWD group,the DX treatment group had significant improvements in neuronal nuclear pyknosis,cell swelling,and tissue matrix laxity after injury.AQP4 positive expression was observed in the hippocampus of rats in the TBI+SWD group and the DX treatment group at each time point after injury.Conclusion Low-dose DX can improve the brain edema of the rats with trauma brain injury complicated by seawater drowning,but the brain protection effect may be limited.
