摘要
目的运用欧洲医药保健网分类系统(PCNE)分类体系点评分析住院病人辅助用药的使用现状,以促进辅助用药的合理使用。方法随机抽取上海交通大学医学院附属新华医院崇明分院2018年10-12月涉及辅助用药的出院病历1 200份,对病历中辅助用药的使用情况进行点评分析,并根据PCNE分类体系对辅助用药医嘱中药物相关问题(DRPs)进行分类汇总。结果发生DRPs的病历数量为317份,共363个DRPs"。治疗安全性""、治疗效果"及"其它方面"三个问题类型数量及占比分别为227(62.53%)、39(10.74%)及97(26.72%)。按原因分类统计,主要集中在"药物选择""、剂量选择"及"治疗疗程"三个方面,数量及占比分别为191(48.11%)、91(22.92%)及75(18.89%)。结论辅助用药临床应用合理性还有待进一步提高。依据PCNE分类体系的解析,需加强向临床反馈目前辅助用药医嘱存在的问题;同时,临床药师应加强干预措施,规范辅助用药临床应用。
Objective To analyze the prescriptions of adjuvant drugs for the inpatients using the classification system of Pharmaceutical Care Network of Europe(PCNE)and promote the rational use of adjuvant prescriptions.MethodsA total of 1200 discharge records involving adjuvant drugs were randomly selected from Chongming branch of Xinhua Hospital Affiliated to the Medical College of Shanghai Jiaotong University from October to December 2018,and the use of adjuvant drugs in the medical records was evaluated.Using the PCNE classification system,the list of drug-related problems(DRPs)in prescriptions was analyzed.ResultsThere were 317 medical records of DRPs,363 cases of DRPs in total.The number and proportion of"treatment safety","treatment effect"and"other aspects"were 227(62.53%),39(10.74%)and 97(26.72%),respectively.The causes of DRPs mainly focused on the three aspects of"drug selection","dose selection"and"treatment course",and the quantity and proportion were 191(48.11%),91(22.92%)and 75(18.89%),respectively.ConclusionsThe rationality of clinical application of auxiliary drugs needs to be further improved.According to the analysis of PCNE classification system,it is necessary to strengthen the feedback to the clinic about the problems existing in the current auxiliary medication orders.At the same time,clinical pharmacists should strengthen the intervention measures,and standardize the clinical application of auxiliary medication.
