摘要
推拿治疗颈椎病主要是改善脊柱关节结构、消除炎症、恢复中枢神经和血流动力学正常功能状态等。推拿手法对脊柱关节结构的影响包括:对神经根压迫的影响,对颈椎排列的影响,对椎动脉供血的影响。推拿治疗颈椎病的手法较为多样,研究重点在于手法改善神经根卡压症状,恢复椎动脉供血之机理方面,针对恢复颈椎椎管狭窄、改善颈髓压迫症状及交感神经刺激症状的机制研究探索相对较为薄弱,神经卡压机制研究的重点在于通过手法整复,改善机械性压迫刺激,而神经电生理方面作用机制研究较少,手法改善椎动脉供血中血流动力学机制研究较多,分子基因学水平研究还远远不够,推拿治疗颈椎病机制研究中缺乏传统中医整体思想及辨证论治、分经论治研究,且针对手法治疗机制的研究以临床及生物力学为主,动物试验较少,今后需加大手法对颈椎椎管狭窄、交感神经型及脊髓型颈椎病治疗机制的研究力度,加强分子基因学方面的研究,全面深入挖掘手法治疗机制,注重对颈椎病患者的手法辨证论治及分经论治等规范研究模式。
The treatment of cervical spondylosis by massage is mainly to improve the structure of spinal joints,eliminate inflammation and restore the normal functional state of central nervous system and hemodynamics. The effects of manipulation on the structure of spinal joints include： the influence on nerve root compression,the influence on cervical vertebrae arrangement,and the influence on vertebral artery blood supply. The technique of massage for the treatment of cervical spondylosis is more varied.The focus of the study is to improve the symptoms of nerve root pressure and to restore the mechanism of the blood supply of the vertebral artery. The research on the mechanism of restoring cervical spinal canal stenosis,improving the compression symptoms of cervical spinal cord and sympathetic nerve stimulation symptoms is relatively weak. The focal point of the study of nerve compression mechanism is to improve the mechanical compression stimulation by means of manipulation,and there is less research on the mechanism of neurophysiology. There are many studies on the mechanism of improving the hemodynamic mechanism in the blood supply of vertebral artery,but the study of molecular genetics is far from enough. In the study of the mechanism of massage for the treatment of cervical spondylosis,there is a lack of the wholism of traditional Chinese medicine and the study of syndrome differentiation and treatment,the study of the mechanism of manipulation is mainly based on clinical and biomechanics,and there are few animal tests. Therefore,in the future,the treatment mechanism of cervical spinal canal stenosis,sympathetic and cervical spondylotic myelopathy needs to be strengthened. Efforts should be made to strengthen the research on molecular genetics,to excavate the mechanism of manipulation and treatment in an all-round way,and to pay attention to the standardized research pattern of the manipulation of cervical spondylosis based on syndrome differentiation,and meridians and the treatment of differentiation and treatmen
