摘要
目的:对比鼻泪管吻合术与改良泪道逆行置管术患者的感受及舒适程度。方法:选取2014年3月~2017年3月我科收治的溢泪、溢脓患者52例,按术式不同分为观察组与对照组,观察组28例(31只眼),行鼻窥镜下改良泪道逆行硅胶置管术;对照组24例(25只眼),行泪囊鼻腔吻合术+泪道再通术,比较两组治愈率、术中出血量、手术时间、手术体验及舒适程度等指标。结果:观察组术中出血量、手术时间均显著低于对照组(P<0.05);观察组在自我感受评价条目中,除术后无溢泪、溢脓及术后泪道冲洗是否通畅外,其余体验及舒适程度均优于对照组;观察组住院时间、治愈率与对照组比较差异无统计学意义(P>0.05)。结论:鼻窥镜引导下改良泪道逆行硅胶置管术创伤更小,手术时间缩短,患者手术体验及舒适程度较高,值得在临床推广应用。
Objective: To compare the feelings and comfort degree of patients with nasolacrimal duct anastomosis and modified lacrimal duct retrograde catheterization. Methods: A total of 52 patients with epiphora and pyorrhea admitted in our hospital from March 2014 to March 2017 were selected and were divided into observation group and control group according to different operations. The observation group with 28 cases( 31 eyes) were treated with modified retrograde catheterization of lacrimal duct under nasal endoscopy,while the control group with 24 cases( 25 eyes) were treated with dacryocystorhinostomy and recanalization of lacrimal duct. The indexes of curative rate,amount of bleeding in operation,operation time,operation experience and comfortable feeling of the two groups were compared. Results: The intra-operative blood loss and operation time of the observation group were significantly lower than those of the control group( P < 0. 05). In the self-feeling evaluation items of the observation group,except for no epiphora,pyorrhea after the operation and the syphilis of lacrimal passages is smooth,the experience and comfortable degree were better than that of the control group. The difference of hospitalization and curative rate of the observation group were of no statistical significance( P > 0. 05). Conclusion: Under the guidance of nasal speculum,modified retrograde catheterization of lacrimal duct had less operation wound and shorten of operation time. The operation experience and comfortable feeling of patients were very high,which is worthy of clinical application.
