腹腔镜联合胆道镜在肝胆管结石再手术中的应用 被引量:34

摘要 目的:探讨腹腔镜联合胆道镜在肝胆管结石再手术中的临床应用价值。方法:回顾性分析2010年1月至2015年12月86例肝胆管结石再手术患者的临床资料,腹腔镜联合胆道镜手术(腔镜组)36例,开腹手术(开腹组)50例,观察分析两组患者的手术时间、术中出血量、术中输血量、结石清除率、术后住院时间、并发症发生率等指标。结果:腔镜组的手术时间、术中出血量、术中输血量、结石清除率、并发症发生率同开腹组相比差异无统计学意义(P>0.05);腔镜组术后住院时间短于开腹组,差异有统计学意义(P<0.05);随访期内两组结石复发率、胆管炎复发率差异无统计学意义(P>0.05)。结论:腹腔镜联合胆道镜用于肝胆管结石再手术安全可靠,疗效满意。 Objective To explore the clinical value of laparoscopy combined with choledochoscopy in repeat surgery for hepatolithiasis. Methods The clinical data on 86 patients who had undergone repeat surgery for hepatolithiasis during January 2010 to December 2015 were retrospectively analyzed. 36 patients received laparoseopy combined with choledochoscopy (laparoscopy group ), while 50 patients received laparotomy (laparotomy group ). Surgical duration, intraoperative blood loss, intraoperative transfusion, stone clearance rates, length of postoperative hospital stay, and rate of complications were observed and analyzed. Results There were no significant differences in surgical duration, intraoperative blood loss, intraoperative transfusion, stone clearance rates, and rate of complications between the two groups (P 〉 0.05). Length of postoperative hospital stay was significantly shorter in the laparoscopy group than in the laparotomy group (P 〈 0.05 ). There was no significant difference in recurrence rates of stone and eholangitis within the follow-up period (P 〉 0.05). Conclusions Use of laparoscopy combined with choledoehoscopy in repeat surgery for hepatolithiasis is safe and feasible and has a satisfactory efficacy.
