摘要
目的比较甲泼尼龙和泼尼松诱导缓解治疗在小儿原发性肾病综合征(PNS)中的应用效果。方法选择2013年6月至2014年6月在青岛市胶州中心医院接受住院治疗的PNS患儿118例,按照随机数字表法随机分为两组,对照组接受泼尼松治疗[2.0 mg/(kg·d)],观察组接受甲泼尼龙治疗[1.8 mg/(kg·d)],比较两组患儿的治疗效果、尿蛋白及血浆白蛋白水平、体液免疫指标等。结果 1观察组患儿接受治疗后的整体疗效优于对照组患儿(P<0.05);2观察组患儿接受治疗后的尿蛋白值低于对照组、血浆白蛋白水平高于对照组患儿[(7.2±0.7)mg/(kg·d)比(21.2±2.3)mg/(kg·d),(47.2±6.0)g/L比(32.6±5.2)g/L,P<0.01],同时尿蛋白转阴时间显著短于对照组患儿[(6.4±0.8)d比(11.6±1.7)d,P<0.01];3观察组患儿接受治疗后的Ig G、C3、C4水平明显高于对照组[(9.3±1.3)g/L比(6.1±0.9)g/L,(1.27±0.18)g/L比(1.13±0.19)g/L,(0.54±0.08)g/L比(0.39±0.07)g/L](P<0.01)。结论甲泼尼龙可以显著提高PNS患儿的治疗效果、降低尿蛋白,同时提升血清白蛋白水平,优化体液免疫指标。
Objective To compare the application value of methylprednisolone and prednisone on chil- dren with primary nephrotic syndrome(PNS). Methods A total of 118 children with PNS were chosen in Jiaozhou Central Hospital from Jun. 2013 to Jun. 2014 as research objects, according to random" number table method divided into control group treated with methyl prednisone [ 2.0 mg/( kg ·d) ], and observation group treated with methylprednisolone[ 1.8 mg/( kg · d) ], and the curative effect, urine protein and serum albu- min levels, humoral immune index of the two groups were compared. Results ① The observation group patients' overall effect was significantly better than the control group (P 〈 0.05 ) ②the observation group patients' urinary protein value was lower than the control group,and serum albumin level was higher than the control group [ (7.2 ± 0.7 ) mg/( kg d) vs (21.2 ±2.3 ) rag/( kg d), (47.2 ± 6.0) g/L vs ( 32.6 ± 5.2 ) g/L,P 〈 0.01 ], at the same time, urine protein turning negative time was significantly shorter than the control group[ (6.4 ±0.8) d vs ( 11.6 ± 1.7 ) d,P 〈 0.01 ] ; )the observation group patients' IgG, C3 and CA levels were significantly higher than the control group [ (9.3 ±1.3 ) g/L vs (6.1± 0.9) g/L, ( 1.27± 0.18) g/L vs (1.13 ±0.19) g/L,(0.54± 0.08) g/L vs (0.39± 0.07) g/L] (P 〈0.01). Conclusion Methylprednisone can significantly improve the effect of the treatment of children with PNS, reduce urinary protein,improve the level of serum albumin and optimize the humoral immune indexes. ; Prednisone
