探究3.0T磁共振T2-Mapping成像T2值对膝关节软骨损伤分级诊断的应用价值 被引量:8

摘要 目的探究3. 0T磁共振T2-Mapping成像T2值对膝关节软骨损伤分级诊断的临床应用价值。方法选择2017年3月~2017年9月67例(男35例,平均年龄41. 2岁;女32例,平均年龄41. 5岁)经临床诊断为膝关节骨性关节炎(KOA)的患者为实验组,另外筛选30例(男16例,平均年龄41. 3岁;女14例,平均年龄41. 2岁)健康志愿者为对照组,采用3. 0T MRI膝关节常规扫描及T2-Mapping序列成像,分别测量膝关节股骨内外侧髁及胫骨内外侧软骨区T2值,行统计学分析。结果 (1)膝关节骨性关节炎患者不同软骨区域及不同损伤分级平均T2值均高于正常膝关节相应软骨区域T2值,差异有统计学意义(P <0. 05);(2)膝关节骨性关节炎患者每一软骨区域损伤Ⅰ~Ⅳ级间平均T2值两两比较,差异无统计学意义(P> 0. 05)。结论膝关节软骨损伤早期即表现为T2值增大,T2-Mapping成像可以发现无明显形态学改变的早期损伤,为临床早期诊治骨性关节炎提供重要参考依据。 Objective To explore the clinical value of 3. 0 T magnetic resonance T2-Mapping combined with routine sequence in the early diagnosis of knee articular cartilage injury by the T2 values of normal knee cartilage and T2 values of different injury grades of knee osteoarthritis patients were measured respectively,and comparing their differences. Methods In this retrospective study,67 patients with knee joint trauma or osteoarthritis who underwent not only knee joint MRI but also arthroscopic surgery from March 2017 to September 2017 in our hospital were included and 30 healthy volunteers. T2-Mapping combined with conventional sequence imaging was used to measure the T2 value of the articular cartilage in the femur and tibia. Results The T2 value of the tibia and the different regions of the femur in the patients with osteoarthritis of the knee was higher than that of the corresponding cartilage region of the normal knee joint( P < 0. 05),and the difference was statistically significant( P < 0. 05). In the ICRS injury classification of the patients with osteoarthritis of the knee,there was a significant difference in the T2 value of the grade I injury of the cartilage in the medial and lateral part of the femur and the medial tibial area with the T2 value of the normal knee cartilage( P < 0. 05). Conclusion In the evaluation of knee cartilage injury,the sensitivity of T2-mapping sequence were higher than that of conventional sequence,the measurement of T2 value can evaluate the degree of cartilage damage of the knee joint,and can indicate early articular cartilage injury without morphology change,which has a high clinical value to diagnose and treat early cartilage injury.
