摘要
目的:总结国内外肿瘤疫苗的临床研究现状。方法:以"肿瘤细胞疫苗、肽疫苗和树突状细胞疫苗"等为关键词,计算机检索CNKI及PubMed数据库,检索年限为2002-2013。纳入标准:1)肿瘤疫苗处于临床试验阶段;2)肿瘤疫苗用于治疗黑色素瘤、肺癌和卵巢癌等实体瘤;3)肿瘤疫苗的靶点是新发现的或者正处于研究的热点。剔除标准:肿瘤疫苗仍处于动物实验阶段。最终纳入46篇文献。结果:大量的肿瘤疫苗已经投入到临床试验,用于实体瘤的治疗并显示出各自的特点。肿瘤细胞疫苗免疫原性较低,常需与佐剂联用才能有效诱导免疫应答;肽疫苗易于合成,靶向性强,但具有MHC限制性,能诱导特异性T细胞免疫应答;DC疫苗作为一种融合疫苗,显示出强烈的免疫原性及抗瘤活性;核酸疫苗的可修改性强,能激活特异性的体液免疫反应;抗ID-抗体疫苗制备繁琐而产量高,免疫反应强,安全性好。结论:目前取得突破性进展的肿瘤疫苗很少,但其作为肿瘤免疫治疗的方案具有极大的研究价值。特别是采用融合疫苗能更有效地激发患者的抗瘤免疫反应,改善临床效应。
OBJECTIVE: To review the recent clinical studies of cancer vaccines. METHODS: By cancer cell vaccine, peptide vaccine,dendritic cell vaccine etc as keywords, the papers of literature were collected by electronically searching CNKI and Pubmed (2002--2013). Retrieval criteria: 1) The cancer vaccines were in clinical trials;2) The cancer vaccines were used for the treatment of melanoma,lung cancer,ovarian cancer or other solid tumors; 3) The targets of cancer vac- cines were newly discovered or in hot researches. Exclusion criteria:The cancer vaccines were still in animal studies. Forty- six articles were selected for inclusion of the review. RESULTS: Large numbers of cancer vaccines have been put into clini- cal trials for the treatment of solid tumors,showing their own advantages and disadvantages. Due to lower immunogenici- ty, tumor cell vaccines are often used in combination with adjuvants to induce immune response effectively. Peptide vac- cines are synthesized easily with strong targeting property and MHC restriction. They are able to induce specific T cell im- mune response. As a kind of fusion vaccines,dendritic cell vaccines show strong immunogenicity and anti-tumor activity. DNA vaccines can be modified to meet the needs of researchers and activate specific humoral immune response. Anti-idio- type-antibody vaccines are produced complicatedly with high yield, showing strong immune response and good security. CONCLUSIONS:Until now,the cancer vaccines which have got a breakthrough are rare. However,they are valuable in re- searches as the way of cancer immunotherapy. Especially using fused vaccines can stimulate anti-tumor immune response and improve the clinical effect more effectively.
