鼻腔结构重建术用于悬雍垂腭咽成形术后无效患者的治疗 被引量:1

Nasal cavity structure reconstruction surgery on obstructive sleep apnea hypopnea syndrome patients without improvement after uvulopalatopharyngoplasty

摘要 目的探讨鼻腔结构重建术治疗悬雍垂腭咽成形术(UPPP)后无效的阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者的疗效。方法选择35例UPPP后无效的OSAHS患者接受鼻腔结构重建术治疗。比较治疗前、后呼吸暂停低通气指数(AHI)、最低血氧饱和度(LSaO2)、鼾声评级、Epworth嗜睡量表(Epworth sleepinessscore,ESS)、鼻阻力和视觉模拟量表(visual analoguescale,VAS)评分,评价治疗效果。结果治疗总有效率为65.7%。患者治疗后6个月,鼾声评级的降低较治疗前无显著差异,ESS评分较治疗前降低非常明显(P<0.01),AHI较治疗前减低明显(P<0.05),LSaO2较治疗前升高明显(P<0.05),鼻阻力和VAS评分均明显减小(P<0.05)。结论对UPPP后无效的OSAHS患者,鼻腔结构重建术是一种有效的补救性治疗方法。 OBJECTIVE To investigate the curative effect of nasal cavity structure reconstruction surgery on ineffective obstructive sleep apnea hypopnea syndrome (OSAHS) after uvulopalatopharyngoplasty (UPPP) . METHODS there were 35 OSAHS patients without any improvement after UPPP were choosed to undergo nasal cavity expansion surgery. Preoperative apnea hypopnea index (AHI) , lowest saturation of blood oxygen (LSaO2) , snoring scale, Epworth sleepiness scale (ESS) , nasal airway resistance (NAR) and visual analogue scale (VAS) were compared with those 6 months after nasal operation to evaluate the curative effect of nasal cavity structure reconstruction surgery. RESULTS The effective rate was 65.7%. At 6 months after operation, the ESS decreased much significantly (P〈0.01) , there was no difference on snoring scale. LSaO2 improved significantly (P〈0.05) , and AHI decreased significantly (P〈0.05) . In addition, NAR and VAS scores decreased significantly (.P〈0.05) . CONCLUSION Nasat cavity expansion surgery is an effective treatment to OSAHS patients without improvement after UPPP.
