摘要
目的探讨联合检测血清β-HCG和孕酮对宫外孕的早期诊断价值。方法选取收治入院的25名宫外孕患者作为实验组,同期30名正常妊娠的妇女作为对照组,比较两组对象血清中孕酮和β-HCG间隔48h值增长率,再结合宫外孕诊断的金标准,计算联合检测血清β-HCG和孕酮诊断宫外孕的敏感度、特异度、准确度。结果宫外孕组和正常早孕组在孕酮([9.6±2.78)vs(37.9±6.63)]和β-HCG间隔48h值增长率([14±4.68)%vs(123±5.84)%]上的差别具有统计学意义(P<0.001),单用β-HCG诊断宫外孕的准确度为83.6%,单用孕酮的准确度为70.9%,而联合β-HCG和孕酮诊断宫外孕的准确度可达94.5%,三种方法准确度不全相同,差异有显著性(χ2=10.626,P=0.005),联合β-HCG和孕酮诊断宫外孕的准确度明显高于孕酮(χ2=10.752,P=0.001)。结论联合检测血清β-HCG和孕酮是一个对宫外孕的诊断具有敏感性和特异性的指标。
Objective To explore the significance of the co-detection of serum progesterone and β-HCG in the early diagnosis of ectopic pregnancy. Methods We selected 25 patients admitted to our hospital as a trial group, 30 intrauterine pregnancy women as a control group, and the serum progesterone and 48h interval increasing rate of β-HCG were compared between the two groups. By combined the hormone results with gold standard for ectopic pregnancy, we calculated the sensitivity, specificity and accuracy of this method. Results There was a significant difference in progesterone[（9.6 ＋ 2.78）vs（37.9 ± 6.63 ）] and β-HCG[（ 14 ± 4.68）%vs（ 123 ± 5.84）%] between the two groups. The accuracy of β -HCG diagnosis alone was 83.6%, and the accuracy of progesterone alone was 70.9% , while the accuracy of combining β -HCG with progesterone in the diagnosis reached 94.5%, significantly higher than that of progesterone alone（x 2=10.626, P=0.005）, with a significant difference among three groups（ X 2=10.626, P=0.005）. Conclusion The co-detection of the serum β -HCG and progesterone is a marker with high sensitivity and specificity for the diagnosis of ectopic pregnancy.
