煤工尘肺患者血清白细胞介素-8、-13和-18水平的变化 被引量:6

Changes of IL-8,IL-13 and IL-18 levels in serum of coal worker's pneumoconiosis

摘要 目的探讨血清白细胞介素(IL)-8、IL-13和IL-18水平在煤工尘肺(CWP)发生发展中的作用。方法采用酶联免疫吸附双抗体夹心测定(ELISA)方法检测了349例CWP患者(CWP组,Ⅰ期322例,Ⅱ+Ⅲ期27例)、57例0+(0+组)、64例具有相同接尘史但未患尘肺的井下煤矿工人(接尘对照组)及50例井上体格检查健康人员(正常对照组)的血清IL-8、IL-13和IL-18的水平。结果与正常对照组比较,血清IL-18水平在接尘对照组和0+组显著降低(P<0.05);与接尘对照组和0+组比较,CWP组显著增高(P<0.05)。与正常对照组比较,IL-13水平在接尘对照组、0+组和CWP组均显著增高(P<0.05)。与正常对照组比较,血清IL-8水平在接尘对照组和0+组显著降低(P<0.05);与接尘对照组和0+组比较,CWP组显著增高(P<0.05)。与Ⅰ期CWP组比较,Ⅱ+Ⅲ期CWP组患者血清IL-8、IL-13和IL-18水平差异无统计学意义(P>0.05)。CWP组不同接尘年限比较,3项指标水平均未发现显著改变(P>0.05)。随着CWP患者年龄的增加,IL-8显著增加(P<0.05)。与单纯CWP比较,合并肺部疾病组及合并心脑血管疾病组IL-8水平显著增高(P<0.05)。IL-18和IL-8在正常对照组、0+组和CWP组呈正相关。结论 IL-8、IL-13和IL-18可能在CWP的发病过程中起一定作用,检测血清3项指标可能对CWP早期诊断和病情监测有一定价值。 Objective To explore the changes of serum interleukin(IL)-8,IL-13 and IL-18 and its possible role in the occurrence and development of coal worker's pneumoconiosis(CWP).Methods Serum levels of IL-8,IL-13 and IL-18 were determined in 349 cases of CWP(CWP group),57 cases of 0+miner(0+ group),64 cases of healthy miner(dust exposed control group) and 50 cases healthy control above the mine(control group) by a enzyme-linked immunosorbent assay(ELISA).Results Compared with the control group,serum level of IL-18 in dust exposed control group and 0+ group was significantly lower(P〈0.05).Serum level of IL-18 in CWP group was significantly higher than those in dust exposed control group and 0+ group(P〈0.05).Compared with the control group,serum level of IL-13 in dust exposed control group and 0+ group and CWP group were significantly higher(P〈0.05),while serum level of IL-8 in dust exposed control group and 0+ group were significantly lower(P〈0.05).Compared with dust exposed control group and 0+ group,serum level of IL-8 in CWP group was significantly higher(P〈0.05).There were no significant differences in serum levels of IL-8,IL-13 and IL-18 between CWP stage Ⅰ,Ⅱ and Ⅲ(P〉0.05).There were no significant differences between the three parameters and the duration of exposure to dust in CWP group(P〉0.05) either.With the increasing of age,the level of IL-8 in CWP group increased(P〈0.05).Serum level of IL-8 in CWP with lung disease group or with heart and brain vascular disease group were significantly higher than that in CWP simply(P〈0.05).There was a positive correlation between serum levels of IL-18 and IL-8 in control group,0+ group and CWP group.Conclusion IL-8,IL-13 and IL-18 may play an important role in the pathogenesis of coal worker's pneumoconiosis.The detection of the three parameters in serum may be used in the early diagnosis and the survey of therapeutic effect of coal worker's pneumoconiosis.
