摘要
目的从文献计量学角度初步分析中国儿童尿道损伤(CUI)文献现状,为进一步研究与临床实践提供参考。方法依据循证医学原理与系统评价方法,全面检索中国生物医学文献数据库截至2008年9月的CUI相关研究文献,采用JBI叙述性研究质量评价方法评估纳入文献质量,提取有效数据后以Excel2003建立数据库,对纳入研究文献的研究特征、研究质量、研究内容、病例特征、诊断及处理、结局评价及随访等项目进行统计分析。结果中国CUI总文献量为266篇,与CUI研究相关文献共计91篇;84篇文献均为叙述性研究,61篇为病例分析,治疗性研究占大多数,总计80篇(95.24%);≥25例文献22篇;纳入研究大多数方法学质量较低,CUI以意外伤致骨盆骨折或骑跨伤为主;39篇文献(46.43%)采用尿道造影进行诊断,治疗多数侧重于CUI后期尿道修复重建或并发症的处理(23篇,占28.75%);仅17篇文献(20.24%)有客观疗效评价指标,包括尿道造影(6篇)、尿动力学检测(9篇)、残余尿测定(7篇)等。18篇文献(21.43%)报道了术后随访,随访2个月至21年;并发症主要涉及尿道狭窄、尿瘘形成、尿失禁及性功能障碍等。结论我国CUI相关研究较少,大多数研究质量低,CUI的相关研究需进一步加强。
Objective To investigate the development of children urethral injury（CUI）research in China based on evidence-based medicine principles and bibliometric approach.Methods We searched the literatures about CUI research updated until September 8th 2008 from Chinese Biomedicine Database （CBM）.The Joanna Briggs Institute （JBI） approach was used to evaluate the literature quality.Database was established with Excel 2003 for the eligible literatures.Research characteristics,research quality,research contents,case characteristics,diagnosis and treatments,outcome evaluation and follow-up of the eligible literatures were analyzed.Results A total of 266 Chinese CUI papers were included,91 accord with the criteria of our research.84 papers were descriptive studies,61 papers were cases analysis,80 papers （95.24%） were surgery intervention studies.There were 22 papers with case numbers over or equal to 25.Most of the studies were of low quality.The causes of CUI were mainly pelvic fracture and straddle injury.39 papers （46.43%） reported the diagnosis by urethrography,the treatments mostly focused on the post-urethral reconstruction of CUI or complications treatment （23 papers,28.75%）.Only 17 papers （20.24%） reported objective efficacy evaluation indexes,included urethrography （6 papers）,urodynamics study （9 papers） and residual urine test （7 papers）.18 papers （21.43%） reported the follow-up,lasting from 2 months to 21 years.Complications mainly related to urethral stricture,urinary fistula,urinary incontinence and sexual dysfunction.Conclusions CUI researches in China are of small amount,mostly with low quality and need to be further strengthened.
