ABCD评分对短暂性脑缺血发作短期预后的急诊评估价值 被引量:22

摘要 目的评价ABCD评分对短暂性脑缺血发作(TIA)后7d和90d脑梗死发生的急诊评估价值。方法以2006—01~2007—06住院的TIA患者为研究对象,收集其临床资料、磁共振弥散加权成像(DWI)和磁共振血管成像(MRA)的检查结果,按ABCD评分标准完成评分。随访TIA后7d和90d脑梗死的发生率,并分析ABCD评分与TIA后短期脑梗死发生的关系。结果TIA后90d共35例(30.7%)发生脑梗死,其中24例(21.1%)发生于7d内;ABCD评分与TIA后7d和90d脑梗死发生之间有直线相关关系,ABCD评分越高,脑梗死的发生率越高(P〈0.001);多因素Logistic回归分析发现,ABCD评分(OR2.80.95%CI1.37~5.68,P=0.005)是TIA后7d脑梗死发生的独立危险因素,ABCD评分(OR2.64.95%CI1.39~4.93,P=0.003)和DWI异常(OR5.84.95%CI1.08~31.66,P=0.041)是TIA后90d脑梗死发生的独立危险因素。结论ABCD评分是急诊预测TIA短期预后的一种简单、有效的方法,结合DWI结果可以提高其对TIA后90d脑梗死的预测价值。 Objective To evaluate the ability of ABCD score in predicting cerebral infarction of transient isehemie attack ( TIA) patients at 7th and 90th day. Methods Clinical data, diffusion - weighted imaging ( DWI) and magnetic resonance angiography (MRA) findings were collected in a cohort of hospitalized TIA patients from January 2006 to June 2007. The ABCD score was applied to all the patients and used to stratify the 7 - day and 90 - day cerebral infarction risk. Results A total of 35 ( 30.7% ) patients had cerebral infarction within 90 days after symptoms onset, while 24 (21. 1% )patients occurred within 7 days. Linear correlation existed between ABCD score and short - term risk of cerebral infarction in TIA patients( P 〈0.001). The higher the score was, the higher the incidence was. By multiple Logistic regression analysis, ABCD score ( odds ratio[ OR] ,2.80; 95% confidence interval [ CI], 1.37 - 5.68 ; P = 0. 005 ) was an independent predictor of 7 - day risk of cerebral infarction. At the same time, ABCD score (0R,2.64; 95% CI, 1.39 -4.93; P =0. 003) and DWI abnormality ( OR, 5.84; 95% CI, 1.08- 31.66; P = 0. 041 ) were independently associated with 90 -day risk of cerebral infarction. Conclusions The ABCD score is simple and effective to predict short - term risk of cerebral infarction in the patients with TIA. Combination with the information of DWI can improve the predictive accuracy of 90 - day risk of cerebral infarction after TIA.
