术前CT测量对颈椎椎弓根形态个体化评价的意义 被引量:19

摘要 目的:探讨颈椎椎弓根螺钉内固定术前CT测量对个体化椎弓根形态评价的意义。方法:20例颈椎标本(C3~C7),分别用CT测量及手工测量两种方法对椎弓根的形态进行评价,椎弓根形态的测量包括椎弓根长度、宽度、高度、投射角度、椎弓根轴线在关节凸上的投射点到关节凸外侧缘及上缘的距离,以及投射点是否投射到椎弓根在关节凸的投射面积之内。结果:C3~C7椎弓根全长、宽度、高度、内偏角度及头/尾偏角度分别为27.3~33.5mm、5.2~6.6mm、7.4-8.5mm、36.3°-49.7°及-12.7°~13.3°,椎弓根形态测量的两组数值无显著性差异,CT测量决定的进钉点均位于椎弓根在关节凸的投射面积之内。结论:颈椎椎弓根形态变异大,术前CT测量能提供椎弓根螺钉内固定所需的个体化数据。 Objectives: To explore effects of preoperative CT measurement on individual assessment of cervical pedicle. Methods: Pedicles from 20 human cadaveric vertebrae specimens (C3-7) were evaluated adopting computed tomography film and manual measurement respectively, referneces dealing with pedicle length/width/high, projecting angle, the distance between the projecting point of the pedicle axis on the lateral mass to the lateral and superior edge of the lateral mass. We also probed whether the projecting point decided by the measurement can project into the area of the pedicle on the lateral mass. Results: The total pedicle length/width/high ,medial and cephalad or caudal angle of the pedicle from C3 to C7 were 27.3-33.5 mm, 5.2-6.6 mm, 7,4-8.5 mm, 36.3°-49.7° and -12.7°-13.3° respectively. There were no significant difference among the data obtained by CT scan and manual measurement, All projecting points of pedicles axes decided by the CT measurement projected into the area of the pedicles on the lateral mass. Conclusions: CT measurements can provide individual quantitative data for transpedicular screw fixation in the cervical spine.
