摘要 目的探讨在全自动生化仪上直接测定葡萄糖六磷酸盐脱氢酶(G6PD)活性对检测G6PD缺乏的可靠性。方法同时用生化仪上直接测定G6PD活性、G6PD/6PGD比值法及高铁血红蛋白还原率法(MHB-RT)对246例成人标本进行检测分析。结果MHB-RT阳性检出率最高为8.13%,直接酶活性法与G6PD/6PGD比值法有较好的符合率,二者对G6PD缺乏的检出率同为6.1%,但2例用比值法测定结果为可疑的女性样本(高度怀疑为杂合子),直接酶活性法测定结果分别为1613、1669μ/L,在正常参考范围的低值。结论在全自动生化仪上直接测定G6PD活性,可用于临床G6PD缺乏的筛查,对于女性样本酶活性在1300~2000μ/L者,建议同时用G6PD/6PGD比值法检测,以提高杂合子的检出率。 Objective To investigate dependability of detecting glucose-6-phosphate(G6PD) activity by biochemistry analyzer for examining G6PD deficiency. Methods 246 adult blood samples Were detected by biochemistry analyzer, G6PD/6GPD ratio test and methemoglobin reduction test (MHB-RT) simultaneously. Results The highest detection rate was 8.13% with the method of MHB-RT. There was a high accordance rate between biochemistry analyzer detection and G6PD/6GPD ratio test. Both detection rates were 6.1%. But two female cases' results detected with the method of G6PD/6GPD ratio test were doubtful. They were suspicious of heterozygosity. Meanwhile, their G6PD level was 1 613μ/L and 1 669 μ/L respectively detected by biochemistry analyzer, lying in the lower limit of the reference range. Conclusion Detecting G6PD activity detecting by biochemistry analyzer can be used for clinical filtration of G6PD deficiency. The female samples whose results are in the range of 1 300-2 000 g/L are suggested to be detected with the method of G6PD/6GPD ratio test simultaneously so as to improve the detection rate of heterozygosity.
