摘要
目的运用数据挖掘技术探索杨志旭教授治疗风温肺热病的临床用药规律。方法搜集杨志旭教授2019年1月1日—2019年12月18日在中国中医科学院西苑医院急诊留观病房查房期间的风温肺热病患者处方,运用TCM Miner、SPSS Statistic 25.0、SPSS Modeler 14.1进行频数分析、关联规则分析、复杂网络分析、聚类分析、贝叶斯网络分析等。结果纳入的222张处方中使用频次≥30次的药物28味,排名前10位的药物分别为黄芪、白术、瓜蒌、生地黄、黄芩、茯苓、地骨皮、白芷、竹茹、当归;关联分析得到"白术-黄芪"等15条2味药物关联;复杂网络分析显示核心药物是黄芪,临证时处方根据情况灵活化裁;聚类分析共得"远志、酸枣仁、石菖蒲、白果、丹参、厚朴"等4个至少含3味中药的聚类处方;贝叶斯网络分析计算了证型与高频药物的概率关系,如正虚邪恋证使用熟地黄、独活、益智仁的概率较高。结论杨志旭授治疗风温肺热病时以补虚扶正、清热化痰、凉血解毒为主要治法。
Objective To explore the prescription regularity of Professor YANG Zhixu in the treatment of wind-warm lung heat disease by data mining technology.Methods The cases of wind-warm lung heat disease treated by Professor YANG Zhixu in the emergency observation wards of Xiyuan Hospital of China Academy of Chinese Medical Science from January 1 st to December 18 th in 2019 were collected.Then the frequency analysis,association analysis,complex network analysis,cluster analysis and bayesian network analysis were performed by TCM Miner,SPSS Statistic 25.0 and SPSS Modeler 14.1.Results Among the 222 included prescriptions,28 Chinese herbs of frequent use(≥30)were involved and the top 10 were listed as follows:Huangqi(Astragali Radix),Baizhu(Atractylodis Macrocephalae Rhizoma),Gualou(Trichosanthis Fructus),Shengdihuang(Rehmanniae Radix),Fuling(Poria),Digupi(Lycii Cortex),Baizhi(Angelicae Dahuricae Radix),Zhuru(Bambusae Caulis in Taenias)and Danggui(Angelica sinensis).Besides,15 herb pairs,such as Baizhu(Atractylodis Macrocephalae Rhizoma)-Huangqi(Astragali Radix),were obtained by the association analysis.Furthermore,the complex network analysis revealed that Huangqi(Astragali Radix)was the core drug and the prescription would be flexibly modified in clinical practice.In addition,the result of cluster analysis showed 4 clustering prescriptions which at least contained 3 Chinese herbs,including Yuanzhi(Polygalae Radix),Suanzaoren(Ziziphi Spinosae Semen),Shichangpu(Acori Tatarinowii Rhizoma),Baiguo(Ginkgo Semen),Danshen(Salviae Miltiorrhizae Radix Et Rhizoma)and Houpo(Magnoliae Officinalis Cortex).Finally the bayesian network analysis was used to calculate the probabilistic relationships between patterns of syndrome and drugs of high-frequency.For example,Shudihuang(Rehmanniae Radix Praeparata),Duhuo(Angelicae Pubescentis Radix)and Yizhiren(Alpiniae Oxyphyllae Fructus)were frequently-used in the syndrome of lingering pathogen due to deficient vital Qi.Conclusion The main strategies of Professor YANG Zhixu in the treatment o
