心电图QRS波变化与冠心病心力衰竭患者病情相关性分析 被引量:5

Analysis of relationship between the changes of QRS wave in ECG and the severity of heart failure in patients with coronary heart disease

摘要 目的心电图检查是诊断冠心病心力衰竭的重要方式,心电图QRS波变化可反映心室传导情况。本研究分析心电图QRS波变化与冠心病心力衰竭患者病情程度的相关性。方法选取2018-02-01—2019-01-31西华县人民医院收治的88例冠心病心力衰竭患者作为研究对象,按照QRS波群时延分为<120ms组与≥120ms组,各44例。对比两组患者心功能分级、左室射血分数(left ventricular ejection fraction,LVEF)、左室短轴缩短分数(left ventricular fractional shortening,LVFS)和左室舒张末期内径(left ventricular end-diastolic dimension,LVEDd),分析心电图QRS波群与心功能分级、LVEF、LVFS、LVEDd的关联性。结果<120ms组心功能分级Ⅱ级14例,Ⅲ级18例,Ⅳ级12例,≥120ms组Ⅱ级8例,Ⅲ级13例,Ⅳ级23例,<120ms组心功能分级低于≥120ms组,u=2.191,P=0.029;<120ms组LVEF为(53.17±14.19)%,高于≥120 ms组的(40.36±12.87)%,t=4.436,P<0.001;<120 ms组LVFS为(26.91±7.26)%,高于≥120ms组的(20.48±6.45)%,t=4.392,P<0.001。<120ms组LVEDd为(58.26±11.69)%,低于≥120ms组的(67.17±12.85)%,t=3.402,P=0.001。Spearman相关性分析结果显示,QRS波群时限延长与冠心病心力衰竭患者心功能分级(r=0.713,P<0.001)和LVEDd(r=0.695,P=0.004)呈正相关,与患者LVEF(r=-0.733,P=0.001)和LVFS(r=-0.706,P=0.002)呈负相关。结论QRS波时限延长越长,冠心病心力衰竭患者心功能分级和LVEDd越高,LVEF和LVFS越低。QRS波时限延长与冠心病心力衰竭患者心功能分级与LVEDd呈正相关,与LVEF和LVFS呈负相关,临床可据此判断患者病情,为制定治疗方案提供参考依据。 OBJECTIVE The electrocardiogram(ECG)is an important way to diagnose coronary heart failure(CHF).The changes of QRS wave in ECG can reflect the condition of ventricular conduction.This study analyzes the correlation between the changes of QRS wave in ECG and the severity of heart failure in patients with coronary heart disease.METHODS From February 1,2018 to January 31,2019,a total of 88 patients with CHF were divided into<120 ms group(n=44)and≥120 ms group(n=44)according to QRS group delay.The cardiac function grading,left ventricular ejection fraction(LVEF),left ventricular fractional shortening(LVFS),left ventricular end-diastolic dimension(LVEDd)were compared between the two groups,and the QRS wave group,cardiac function grading,and the relevance of LVEF,LVFS and LVEDd were analyzed.RESULTS There were 14 cases of gradeⅡ,18 cases of gradeⅡ,12 cases of gradeⅣin<120 ms group,8 cases of gradeⅡ,13 cases of gradeⅢand 23 cases of gradeⅣin≥120 ms group.And the cardiac function grade in<120 ms group was lower than that in≥120 ms group(u=2.191,P=0.029).The LVEF and LVFS in<120 ms group were(53.17±14.19)%and(26.91±7.26)%,respectively,higher than those in>120 ms group(40.36±12.87)%and(20.48±6.45)%,respectively,with t values of 4.436 and 4.392,both P<0.001.The LVEDd was(58.26±11.69)%,lower than that in>120 ms group(67.17±12.85)%,t=3.402,P=0.001.Spearman correlation analysis showed that the prolongation of QRS wave group duration was positively correlated with cardiac function classification(r=0.713,P<0.001)and LVEDd(r=0.695,P=0.004),negatively correlated with LVEF(r=0.733,P=0.001)and LVFS(r=-0.706,P=0.002).CONCLUSIONS The longer the QRS wave duration is,the higher the heart function classification and LVEDd are,the lower the LVEF and LVFS are.The prolongation of QRS wave durationis positively correlated with cardiac function classification and LVEDd,and negatively correlated with LVEF and LVFS in patients with coronary heart disease and CHF,which can be used to judge the patients′condition and pro
